About the Role

The Leeds Road Practice is seeking a highly skilled Clinical Pharmacist to join our team. As a Clinical Pharmacist, you will play a key role in providing expert medicines advice and support to patients and healthcare professionals.

Key Responsibilities

  • Work as part of a multidisciplinary team to clinically assess and treat patients using your expert knowledge of medicines.
  • Take responsibility for the care management of patients with chronic diseases and undertake clinical medication reviews.
  • Provide specialist expertise in the use of medicines and help address public health and social care needs.
  • Lead on person-centred medicines optimisation and quality improvement initiatives.
  • Support patients to take their medications effectively and promote self-care.
  • Develop relationships with other pharmacy professionals and healthcare teams to improve patient outcomes.
  • Take a central role in shared care protocols, clinical research, and liaison with specialist pharmacists.

Person Specification

Essential:

  • Registered Pharmacist with GPhC
  • Masters in Pharmacy degree

Desirable:

  • Independent prescribing
  • Membership Primary Care Pharmacy Association (PCPA)
  • Specialist knowledge through a postgraduate diploma or equivalent

Other Requirements

Essential:

  • Successful Disclosure Barring Service (DBS) check
  • Maintain confidentiality at all times
  • Full UK driving licence

Desirable:

  • Post registration experience
  • Recent primary care or applicable experience
  • Use of SystmOne or other clinical system
