Home Care Assistant

Lincolnshire Community Health Service NHS Trust is seeking a Home Care Assistant to join our team. In this role, you will provide high-quality care and support to patients in their own homes, promoting independence and preventing dependence.

You will work closely with registered therapists to assess and rehabilitate patients post-discharge from hospital, providing reablement care and supporting individuals in achieving their goals.

The salary for this position is estimated to be between £24,000 - £26,000 per year, depending on location and experience.

Responsibilities:
  • Provide reablement care to patients in their own homes
  • Work closely with registered therapists to assess and rehabilitate patients post-discharge from hospital
  • Support individuals in achieving their goals and promoting independence

Please note that all staff are required to access organisational policies, as well as Local Safeguarding Children Board and Local Safeguarding Adults Board policies and procedures.
