Main Responsibilities:

  • Facilitate referrals and provide advice, guidance, and structured interventions to individuals in need.
  • Engage key stakeholders and recruit community partners to promote the charity's services.

Requirements:

  • A Level 4 qualification in a relevant field, such as health and social care.
  • Experience working in a multi-disciplinary setting and with vulnerable populations.
  • Ability to travel independently across the North and East Yorkshire region.
