We offer a competitive salary of £43,041 - £51,668 per annum, depending on experience, plus a range of benefits including generous annual leave entitlement, flexible working opportunities, and access to continued professional development.

Job Summary

  • To provide intensive home-based treatment and support to older adults with mental health needs
  • To work collaboratively with multidisciplinary teams to develop and implement individualized care plans
  • To maintain accurate and up-to-date records of service user interactions

Main Duties and Responsibilities

  • Assessing the mental and physical health needs of service users, including risk assessments
  • Developing and implementing individualized packages of care, including signposting when necessary
  • Providing intensive home-based treatment and support to service users
  • Liaising and negotiating with other professions, statutory and independent agencies to ensure the best possible service is provided to service users and carers

Required Skills and Qualifications

  • RNMH, Occupational Therapist or Social Worker qualification
  • Completion of Preparation for Mentorship/Supervisor/Assessor Training or willingness to complete Supervisor/Assessor Training
  • Evidence of ongoing Continued Professional Development
