Job Title: Specialist Community Practitioner Team Lead

City Health Care Partnership CIC is seeking a highly skilled and experienced Specialist Community Practitioner Team Lead to join our Jean Bishop Frailty Community Services team.

Main Responsibilities:
  • Provide urgent medical care to frail patients in their usual residence, including care homes.
  • Work as an expert autonomous practitioner, assessing and delivering care and treatment to older people and those with frailty.
  • Collaborate with the multi-disciplinary team and community services to ensure patients receive holistic and comprehensive care and support.
Requirements:
  • Independent non-medical prescriber V300 qualification.
  • Significant post-registration experience in planning and providing holistic care to patients in the community.
  • Experience as an autonomous practitioner and collaborative working across organisations.
  • Knowledge of current legislation and impact on community services, evidence-based practice, and virtual ward concept.
Desirable:
  • Mentorship/Practice Assessor qualification and evidence of supervision/teaching/team management.
