Main Responsibilities:

  • Assessment and Treatment: Conduct thorough assessments of service users and carers with complex needs, identifying areas for improvement and developing personalized treatment plans.
  • Manual Handling and Single-Handed Care: Provide expert advice and intervention regarding complex manual handling situations, ensuring the safety and well-being of patients and staff.
  • Ward-Based Rehabilitation and Assessment: Assess and treat patients requiring ward-based rehabilitation and assessment for discharge planning, working closely with multidisciplinary teams to ensure seamless care.
  • Collaboration and Partnership: Work alongside our Team Manager and partner agencies to ensure effective collaboration and coordination of care.
  • Blue Badge Applications: Undertake assessments and documentation associated with Blue Badge applications, providing expert advice and guidance to patients and families.
  • Functional Assessment and Disability Support: Provide expert advice and support to patients and families, working with people with disabilities and vulnerable individuals to promote independence and well-being.
  • Equipment and Home Modifications: Recommend and provide assessments for equipment and home modifications, ensuring patients have access to the resources they need to live safely and independently.
  • Safeguarding and Dignity: Demonstrate a strong understanding of safeguarding principles and promote dignity and respect in all interactions with patients and families.

To be successful in this role, you will possess:

  • Strong Assessment and Treatment Skills: Ability to conduct thorough assessments and develop effective treatment plans.
  • Expertise in Manual Handling and Single-Handed Care: Knowledge and experience in providing expert advice and intervention regarding complex manual handling situations.
  • Strong Communication and Collaboration Skills: Ability to work effectively with multidisciplinary teams and partner agencies to ensure seamless care.
  • Strong Analytical and Problem-Solving Skills: Ability to analyze complex situations and develop effective solutions.
