About the Role

We are seeking a highly skilled and dedicated Community Staff Nurse to join our team at Sussex Community NHS Foundation Trust. As a Community Staff Nurse, you will play a vital role in delivering high-quality care to patients in their own homes, working towards rehabilitation, self-management, and independence throughout the continuum of care.

Key Responsibilities
  • Deliver high-quality care in the community, working with patients towards rehabilitation, self-management, and independence.
  • Participate in a team of colleagues supporting evidence-based care, facilitating staff learning and development.
  • Manage diabetic care, wound management, palliative care, bladder and bowel care, and other continence needs.
  • Work closely with specialist palliative care services, GPs, and other healthcare professionals to provide comprehensive care.
Requirements
  • RN qualification or degree/diploma.
  • ENB 998 or equivalent or work towards.
  • Experience in a community setting.
  • Good numeracy and literacy skills.
  • Good communication and negotiation skills.
  • Demonstrates competency in a range of clinical skills.
  • Knowledge of current local policies informing community health and social care services.
  • A clear understanding of the role of the Community Staff Nurse.
  • Promote independence and choice in undertaking patient care.
  • IT literate or willing to achieve within an agreed period.
  • Knowledge of Clinical Governance, clinical audit, and clinical supervision.
  • Ability to demonstrate knowledge/understanding of boundaries of role.
