Heart Failure Specialist Nurse

We are seeking a highly motivated and experienced nurse to join our heart failure service in Derbyshire. As a Heart Failure Nurse/Heart Failure Specialist Nurse, you will play a crucial role in delivering high-quality care to patients with heart failure, working closely with our multidisciplinary team to optimize treatment and improve patient outcomes.
Main ResponsibilitiesThe successful candidate will be responsible for managing a caseload of heart failure patients, providing education and support to patients and their families, and working collaboratively with other healthcare professionals to ensure seamless care. You will also be involved in the development and implementation of heart failure care pathways and protocols, and contribute to the growth and development of the heart failure service.

Requirements- Registered General Nurse (RGN) with a strong background in community nursing and heart failure care
- Teaching and assessing in clinical practice qualification (e.g. ENB 998, C&G 7307 or equivalent)
- Non-Medical Prescriber V300 qualification (essential for Band 7 post)
- Experience of working with heart failure patients, including assessment, treatment, and education
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals
