Job Overview

An exciting opportunity has arisen for a dynamic and motivated Public Health Nurse to join our team in the 0-19 Children's and Young People Public Health Service. The successful candidate will work in partnership with children, young people, families, primary care, schools, and statutory and voluntary services to facilitate health-enhancing activities and improve health outcomes.

Main Duties

The post holder will contribute to the safe and effective delivery of the 0-19 service, working within the locality and partnering with the 0-19 teams to support county-wide work for children aged 0-8 years. Clinical activities will include assessing, planning, implementing, and evaluating family-focused care under the delegation of a SCPHN Health Visitor/School Health Nurse.

Key Responsibilities
  • Deliver services to children and families within homes and community settings.
  • Undertake Healthy Child Programme developmental and transitional assessments and planning.
  • Develop, participate, and maintain interagency networks.
  • Work with partnership agencies to facilitate health-enhancing activities and improve health outcomes.

The ideal candidate will have:

  • Demonstrates an understanding of evidence-based practice.
  • First Level Registered Nurse or Midwife qualification.
  • Practice assessor/Supervisor qualification.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a team environment.
