We are seeking a Clinical Pharmacy Professional to join our team in Durham. This is an exciting opportunity for clinically focused pharmacists who wish to expand their experience by working on-site within General Practices.

Job Responsibilities
  • Conduct telephone, face-to-face, virtual and domiciliary clinical medication reviews to proactively manage patients with complex polypharmacy.
  • Review the ongoing need for each medication, ensuring monitoring needs are performed and providing opportunities for patients to achieve better health outcomes.
  • Reduce inappropriate poly-pharmacy and wasteful prescribing.
About the Role

The Clinical Pharmacist will work as part of a multi-disciplinary team in a patient-facing role, supporting and working alongside practice staff to help resolve medication-related issues.

Key responsibilities include:

  • Supporting practice staff with prescription and medication queries.
  • Helping to utilise skill mix, improve patient outcomes, and ensure better access to healthcare.
  • Providing expert clinical medicines advice and addressing public and social care needs of patients.

Salary: £55,000 - £70,000 per annum.
