About the Role

We are seeking a Resident Support Coordinator to join our team at HC-One. In this role, you will be responsible for planning and implementing personalized care plans that promote dignity and independence for our residents.

Key Responsibilities
  • Develop and maintain effective relationships with residents and their families
  • Collaborate with colleagues to ensure seamless delivery of care services
  • Monitor and report any changes in resident condition or behavior
  • Participate in safeguarding training and take initiative to prevent abuse and neglect
What We Offer
