About the Role

We are seeking an experienced Team Leader to join our Adult Community Services team in Reading Extra Care. As a Team Leader, you will play a key role in ensuring the smooth operation of our service, providing excellent care to residents, and supporting our Support Workers in their daily tasks.

Key Responsibilities
  • Lead and manage a team of Support Workers to ensure high-quality care is delivered to residents.
  • Develop and implement Support Plans, Risk Assessments, and other necessary paperwork to ensure resident safety and well-being.
  • Work closely with Support Workers to provide direct care visits, monitoring checks, and carer assessments.
  • Support residents in achieving their goals and promoting social inclusion through local trips and activities.
  • Collaborate with the wider Radis team to ensure seamless service delivery and resident satisfaction.

Requirements
  • Minimum 1 year of experience in the health and social care sector.
  • Currently hold or working towards an NVQ Level 3.
  • Ability to work shifts, including 07:00 AM - 02:00 PM and 02:00 PM - 10:00 PM, and alternate weekends.
