Job Summary: We are seeking a highly motivated and compassionate Service Support Worker to join our Health and Local Crisis Response service. As a Service Support Worker, you will be responsible for providing practical and emotional support to service users who may be struggling to do things for themselves due to age, illness, or vulnerability.

Responsibilities:

  • Providing reactive, focused, and flexible support to adults following a hospital stay to enable early discharge.
  • Responding to referrals from Clinicians and Community Health Professionals.
  • Assess service users' needs, complete and follow a support plan for the individual.
  • Supporting service users in the weeks after hospital discharge or to prevent readmission.
  • Taking someone to a hospital appointment that they otherwise couldn't get to or be picking up prescriptions and shopping.

Requirements: To be successful in this role, you will need to have a full UK manual driving licence, a good knowledge of services provided by the NHS and Social Care, and be able to make things great by improving service quality for the benefit of users. You will also need to be professional, diplomatic, and confidential in your dealings with queries.
