About the Role:
We are seeking an experienced and skilled Clinical Nurse Specialist to join our Adult Congenital Heart Disease (ACHD) team at Guy's and St Thomas' NHS Foundation Trust. The successful candidate will provide expert clinical care and support to patients with congenital heart disease, working closely with the multidisciplinary team to ensure seamless care transitions.
Key Responsibilities:
- Provide clinical support and advice to patients with congenital heart disease, including telephone and email advice lines.
- Support the review and management of in-patients, contributing to care plans and communicating with patients and their relatives.
- Act as a resource for healthcare professionals within the Trust and across the network.
- Run face-to-face and virtual clinics for specific patient groups, including transition nurse-led clinics.
- Pre-assess and provide education to patients undergoing intervention or surgery.
- Collaborate with anaesthetists to ensure patient safety and planning of care for complex cases.
- Attend and contribute to multidisciplinary team meetings.
- Provide specialist education and training to nurses, including students, midwives, and allied health professionals.
Person Specification:
- RN1 with BSc first degree.
- Post-registration qualification in a relevant area of specialty or equivalent post-qualification experience.
- Mentorship and leadership courses.
- Non-medical physical assessment course.
Desirable Criteria:
- Master's degree or working towards one.
