Carr Gomm is seeking a skilled and compassionate Support Practitioner to join our Oban Day & Overnight Responder Service. As a key member of our team, you will play a vital role in providing effective and efficient support to individuals in their own homes, reducing the risk of ambulance call-outs and hospital admissions.

Our Responder Service is designed to promote independence and positive outcomes for the people we support. You will work closely with our team to develop and implement personalized support plans, ensuring that each individual receives the care and attention they need to thrive.

Key Responsibilities:

  • Provide high-quality support to individuals in their own homes, promoting independence and well-being
  • Develop and implement personalized support plans, working closely with our team and the individuals we support
  • Collaborate with our Responder Service team to ensure seamless delivery of support
  • Contribute to the growth and development of our Responder Service, identifying opportunities for improvement and innovation

Requirements:

  • Eligibility to work in the UK or ability to apply for a visa granting permission to work in the UK
  • Valid driver's license and access to a vehicle (provided by Carr Gomm)
  • Excellent communication and interpersonal skills
  • Ability to work flexibly, including evenings and weekends
