We are seeking a highly motivated Adult Health Practitioner to join our team in North Liverpool. As a key member of our district nursing team, you will be responsible for delivering high-quality clinical care to patients in their own homes.
The ideal candidate will have a strong understanding of clinical governance, patient group directions, and associated policies. You will be confident in your ability to prioritise health problems and intervene appropriately to assist patients in complex, urgent, or emergency situations.
A full driving license is required for this role, as you will need to travel across boundaries to provide care to patients.
Main Duties- Prioritise health problems and intervene appropriately to assist patients
- Deliver evidenced-based care
- Produce accurate, contemporaneous, and complete records of patient consultations
- Participate in the maintenance of quality governance systems and processes
- Registered Practitioner qualification
- Mentor/teaching qualification or equivalent work-based experience
- Evidence of clinical experience in relevant field
We offer a competitive salary of £32,000 - £39,500 per annum, plus benefits including flexible working requests and access to ongoing training and development opportunities.
