### Job Summary

We are seeking a highly skilled and experienced Neonatal Sister to join our team at King's College Hospital. The successful candidate will be responsible for coordinating the In-reach Clinic, working closely with the senior Neonatal Discharge Planners and community services to actively reduce length of stay. They will also work in partnership with the multidisciplinary team and senior discharge Planners to determine the best place for continuing care of each baby.

### Key Responsibilities

* Coordinate the In-reach Clinic and work closely with the senior Neonatal Discharge Planners and community services to reduce length of stay.
* Work in partnership with the multidisciplinary team and senior discharge Planners to determine the best place for continuing care of each baby.
* Manage patient flow from admission through to discharge while maintaining high quality patient care and efficient use of resources.
* Work closely with the Senior Nurses and senior Discharge Planners to manage patient flow and maintain high quality patient care.
* Act as a positive role model to junior staff members and ensure the management of the special care area is maintained at all times.
* Carry the Unit bleep as required, resolving local problems/ conflicts, keeping managers/others informed and maintaining positive team dynamics in the unit at all times.

### Requirements

* Experience in discharge planning and/or community working.
* Demonstrable interest in and commitment to the professional development of others.
* Ability to work in a fast-paced environment and manage multiple priorities.
* Strong communication and interpersonal skills.
* Ability to work effectively in a team environment.
