About the Role:

  • We are seeking a highly skilled Clinical Social Care Specialist to join our team in Cardiff.

This is an exciting opportunity to work with our experienced and supportive team in a fast-paced environment. As a Clinical Social Care Specialist, you will be responsible for carrying out discharges across multiple hospital sites, ensuring timely and effective patient care.

Key Responsibilities:

  • To plan and coordinate discharges from hospital sites
  • To work effectively with healthcare professionals to ensure seamless transitions of care
  • To maintain accurate records and reports

Requirements:

">
  • A minimum of 6 months experience of working in the UK as a registered social worker
  • A relevant Social Work Qualification
  • Registration with Social Care Wales / Social Work England
  • Enhanced DBS, registered on the update service or be happy to apply for a DBS
  • 3 Years of referencing
  • Driving license with access to a car

Salary:

We offer a competitive salary up to £37 per hour, dependent upon experience and qualifications.
