We are seeking an experienced Advanced Healthcare Assistant to join our team in the Maidstone area. As a valued member of our Community Nursing Team, you will be responsible for delivering high-quality care to patients in their own homes and residential care settings.
The ideal candidate will have a minimum of 2 years' experience working as a Healthcare Assistant or Worker in a health or social care setting. You will possess excellent communication and interpersonal skills, with the ability to work effectively as part of a team.
Key responsibilities will include:
- Providing personalized nursing care to patients at home and in residential care settings
- Monitoring the quality of care and providing feedback to managers
- Working collaboratively with local nursing teams to develop and implement care plans
