Key Responsibilities:
  • Patient Assessment and Management: Conduct comprehensive assessments and develop individualized care plans to optimize blood glucose control and prevent complications.
  • Patient Education and Self-Management: Deliver diabetes education to patients and their families, empowering them to manage their diabetes through self-care strategies.
  • Multidisciplinary Collaboration: Work collaboratively with healthcare professionals to ensure coordinated diabetes care and participate in multidisciplinary team meetings.
  • Complex Case Management: Manage patients with complex diabetes care needs, including those with co-morbidities and complications.
  • Documentation and Reporting: Maintain accurate patient records and provide detailed reports on patient outcomes.
  • Professional Leadership and Supervision: Provide clinical leadership and guidance to junior nurses and engage in continuing professional development.
  • Health Promotion and Disease Prevention: Lead diabetes prevention initiatives in the community, promoting healthy lifestyles and early intervention.
Requirements:
  • Registered Nurse with current registration with the Nursing and Midwifery Council (NMC)
  • Specialist qualification or certification in diabetes care
  • Minimum 3-5 years of post-qualification experience in diabetes management
  • Excellent clinical knowledge of diabetes care and treatment modalities
  • Strong communication, leadership, and interpersonal skills
