Care Coordinator Assistant

We are seeking a highly skilled and motivated Care Coordinator Assistant to join our Community Care Coordination Service. This is an exciting opportunity to make a positive impact on the lives of patients living with frailty and long-term conditions.
In this role, you will work closely with patients, carers, and healthcare professionals to provide comprehensive assessments, develop personalized care plans, and promote independence and dignity. You will also be responsible for undertaking thorough reviews, ordering equipment, and making referrals to enhance patient well-being.
The successful candidate will have excellent organizational skills, ability to manage their time effectively, and communicate effectively with others. They will also possess a good standard of literacy and numeracy, as well as a willingness to learn and develop their skills.
We offer a competitive salary of £28,000 - £34,000 per annum, depending on experience, plus benefits such as season ticket loans, NHS discounts, and excellent training facilities.
