We are seeking a skilled Community Charge Nurse to join our team at North Ayr Health Centre. As a key member of our community health team, you will be responsible for providing high-quality nursing care to individuals affected by homelessness and social deprivation in the Ayr North locality.

Key Responsibilities

  • Conduct comprehensive assessments, develop, and implement holistic care plans for individuals with complex health and social care needs.
  • Supervise and support staff in the delivery of care services, ensuring a safe and respectful environment for all.
  • Collaborate with multidisciplinary teams to address the needs of vulnerable populations, including those affected by trauma and public protection legislation.
  • Develop and maintain relationships with community partners to enhance care coordination and service delivery.

Requirements

  • Registered Nurse with a current NMC PIN.
  • Excellent communication and interpersonal skills, with the ability to work independently and as part of a team.
  • Ability to provide evidence of personal development and a willingness to undertake relevant training.
  • UK driving license and car ownership are essential for this role.
