Job Title: Clinical Pharmacist

We are seeking an experienced Clinical Pharmacist to join our team at Ely PCNs. As a Clinical Pharmacist, you will play a key role in supporting our primary care network and working closely with our healthcare professionals to improve patient outcomes.

Job Summary

The successful candidate will have a strong background in pharmacy practice, with experience in general practice and a passion for medicines optimisation. You will be responsible for conducting medication reviews, providing education and training to practice teams, and working closely with our GP colleagues to improve patient care.

Main Responsibilities
  • Conduct medication reviews and provide recommendations for improvement
  • Provide education and training to practice teams on medicines optimisation and other relevant topics
  • Work closely with GP colleagues to improve patient care and outcomes
  • Support the development and implementation of local and national guidelines and formulary recommendations
  • Monitor practice prescribing and provide feedback to improve prescribing practices
  • Participate in multi-disciplinary team meetings and contribute to the development of care plans
Requirements
  • Registration with the General Pharmaceutical Council
  • Experience in general practice and medicines optimisation
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Excellent analytical and problem-solving skills
