Adult Social Care Professional

The role sits within the Epping LBC Integrated Neighbourhood team, a busy and long-established team working with adults 18+ living with learning disabilities and autism/physical disabilities and sensory impairment/old age frailty.
This position requires strong experience in assessing social care needs of adults who are 18 years and over and providing effective support to individuals using a strength-based approach to enable them to live as independently as possible with more choice and control.
About the RoleYou will work with complex cases to meet assessed needs for our adults, maximising their potential to live independently.
A key responsibility is to assess social care needs using a person-centred and strengths-based approach and contribute to the development of care and support plans for adults.
Responsibilities- Taking part in case conferences, court attendance, meetings, and statutory reviews, proactively taking the lead when a social care perspective is required.
To be successful, you should have a qualified social work background with current Social Work England registration or a qualified Occupational Therapist with HCPC registration.
Post-qualified 2 years of experience is required, along with an in-depth knowledge of complex cases and The Care Act. AMHP, BIA, or PE qualification is also necessary, or securing intention to complete these qualifications.
Requirements- Evidence of continuing professional development and capability of practice in accordance with current professional standards.
- An understanding of the financial implications of commissioned care packages.
- The ability to make clear decisions, highlighting and limiting potential risks associated with intervention.
Travel around Essex is involved in this role. A driving licence and car, or alternative means of meeting mobility requirements, are essential.
