About the Role

We are seeking an Advanced Practice Pharmacist to join our team in the community intermediate care unit. The ideal candidate will have experience in hospital pharmacy and a strong interest in care of the elderly/intermediate care.

The role involves working with patients requiring short-term inpatient care, providing rehabilitation and supporting their recovery. You will work closely with medical, nursing, and other MDT & pharmacy staff to ensure effective communication and collaboration.

Key Responsibilities
  • Develop and implement clinical pharmacy services to support the care of patients in the unit.
  • Provide leadership and guidance to the pharmacy team.
  • Contribute to the development of policies and procedures related to clinical pharmacy services.
  • Work closely with medical and nursing staff to ensure effective communication and collaboration.

Salary Details: The salary for this position will be based on previous experience, with applicants who are currently suitable to practice at a band 7 level considered for a band 7 position. The estimated salary is £65,000 - £85,000 per annum.
