Job Title: Community Registered Nurse

We are seeking a skilled and compassionate Community Registered Nurse to join our team at the Transformation Unit. As a Community Registered Nurse, you will play a vital role in delivering high-quality patient care to housebound patients in Torquay.

Job Summary

This is an exciting opportunity for a registered nurse to work in an integrated team, providing care to patients in their own homes. You will work closely with a multi-disciplinary team, including community nurses, intermediate care staff, and community matrons, to deliver patient-centered care.

Key Responsibilities
  • Deliver high-quality care to housebound patients in Torquay, working as part of a multi-disciplinary team.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Work collaboratively with primary care and secondary care teams to ensure seamless care transitions.
  • Provide education and support to patients, families, and carers to promote independence and self-management.
  • Participate in clinical governance and quality improvement initiatives to ensure high-quality patient care.
Requirements
  • Qualified Registered Nurse with a strong background in community nursing.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work autonomously and as part of a team, with a patient-centered approach to care.
  • Knowledge of community nursing principles and practices, including health promotion, disease prevention, and management of chronic conditions.
  • Experience working in an integrated health and social care environment, with a strong understanding of local and national health policies.
