Job SummaryThis is an exciting opportunity for a registered nurse to work in an integrated team, providing care to patients in their own homes. Key Responsibilities- Deliver high-quality care to housebound patients in Torquay, working as part of a multi-disciplinary team.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Work collaboratively with primary care and secondary care teams to ensure seamless care transitions.
- Provide education and support to patients, families, and carers to promote independence and self-management.

- Qualified Registered Nurse with a strong background in community nursing.
