Community Psychiatric Nurse Leader

We are seeking a highly motivated Community Psychiatric Nurse Leader to join our team at Avon & Wiltshire Mental Health Partnership NHS Trust. As a key member of our liaison team, you will play a crucial role in supporting patients with complex mental health needs and ensuring seamless care transitions between hospitals and community services.
About the RoleThis Band 6 position offers an estimated salary of £44,036 per annum and presents a fantastic opportunity for experienced professionals to take their careers to the next level. As a community psychiatric nurse leader, you will be responsible for conducting comprehensive assessments, developing individualized care plans, and providing education and support to hospital staff.
Key Responsibilities:- Lead the development and implementation of patient-centered care plans in collaboration with clinical teams.
- Conduct thorough risk assessments and provide guidance on discharge planning to optimize patient outcomes.
- Develop and deliver educational programs for nursing and non-nursing staff to enhance their understanding of mental health interventions.

RequirementsTo succeed in this role, you must possess:
- A Registered Nurse (mental health) or Social Worker qualification.
