We are seeking a highly motivated and skilled Apprentice Family Practitioner to join our team at Harford Health Centre. As a key member of our primary care team, you will be responsible for providing high-quality patient care and contributing to the development of our community healthcare services.
Key Responsibilities:
- Respond to Medical Problems: Respond to medical problems presented by temporary residents as required, utilizing your clinical expertise to provide effective care and support.
- Make Autonomous Decisions: Make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other healthcare workers within the Organisation.
- Assess Patient Needs: Assess the healthcare needs of patients with undifferentiated and undiagnosed problems, identifying potential risks and developing effective care plans.
- Screen for Disease Risk Factors: Screen patients for disease risk factors and early signs of illness, providing timely interventions and referrals as needed.
- Develop Care Plans: In consultation with patients and in line with current Practice long-term conditions/disease management protocols, develop care plans for patients, ensuring comprehensive and coordinated care.
- Refer Patients: Refer patients for further opinions and interventions according to Practice policies and protocols, ensuring seamless transitions of care.
- Record Consultation Notes: Record clear and contemporaneous consultation notes to agreed standards, maintaining accurate and up-to-date patient records.
- Prescribe Medications: Compile and issue computer-generated acute and repeat prescriptions, avoiding handwritten prescriptions wherever possible.
Requirements:
- Professional Qualifications: Must possess relevant professional qualifications and certifications in family medicine or a related field.
- Clinical Experience: Must have significant clinical experience in primary care or a related setting, with a strong understanding of community healthcare principles and practices.
- Communication Skills: Must possess excellent communication and interpersonal skills, with the ability to work effectively with patients, colleagues, and other healthcare professionals.
