We are seeking a dedicated Community Healthcare Professional to join our team at Berkshire Healthcare NHS Foundation Trust. This role offers the chance to work in a specialist mental health and community health services trust rated 'outstanding' by the CQC, providing assessment and treatment for patients in Oakwood Unit, a Community Hospital Ward within the Prospect Park Hospital site.

The post involves working both night and day duty, with opportunities to develop your clinical skills and support the Ward Sisters in the assessment, planning, implementation, and evaluation of patient care. You will be part of a Multi-Disciplinary Team that promotes patient independence and discharge, takes part in board rounds and improvement huddles, and communicates with patient's families and carers.

Responsibilities include contributing to the planning, development, and evaluation of the service, having responsibility for defined projects, and working bank holidays/weekends or unsocial hours to meet the needs of the service.
