Key Responsibilities- Provide person-centered support to encourage customers to maximize their potential and independence
- Assess customers' needs and develop tailored support plans
- Act as a first point of contact for customer queries and concerns
- Build strong relationships with customers, promoting personal choice, privacy, and dignity
- Liaise with external agencies to enhance customers' confidence, health, and wellbeing
- Respond to emergency situations and maintain accurate records
- Experience working with adults who have experienced homelessness
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Organizational skills and functional literacy and numeracy
- NVQ3 in Health and Social Care or willingness to work towards
