Advanced Healthcare Specialist

This is an exciting opportunity for a skilled Advanced Nurse Practitioner to join our Primary Integrated Community Services team. As an experienced nurse with an independent prescribing qualification, you will work alongside GPs and other healthcare professionals to provide high-quality care to patients in our community.
About the RoleYou will be responsible for providing holistic care to patients with a range of acute, non-acute, and chronic medical conditions. This will include assessing, diagnosing, and treating patients, as well as referring them to appropriate specialists or agencies when necessary.
As a key member of our team, you will work collaboratively with colleagues to meet the needs of patients and contribute to the development of primary care services in our community. You will also have opportunities to develop your skills and knowledge through training and professional development programs.
