Advanced Clinical Practitioner

We are seeking an experienced Advanced Clinical Practitioner to join our team in providing high-quality, patient-led care to frail and older adults. This is a challenging and rewarding role that requires strong clinical skills, empathy, and a passion for delivering exceptional patient outcomes.
About the RoleThis is a full-time position that will involve working as part of a multi-disciplinary team to provide face-to-face and telephone consultations with patients presenting with a wide range of minor illnesses, urgent, and same-day requests for treatment, and chronic disease management. You will be responsible for assessing patients' medical conditions, developing personalized care plans, and evaluating patient outcomes.
A significant portion of your time will be spent caring for patients in care and nursing homes, where you will assess their medical condition, frailty review, and develop strategies to improve their quality of life.
Key Responsibilities- To provide high-quality, cost-effective services to patients, focusing on frail and older adults, and those with low-level mental health concerns.
- To work as part of a multi-disciplinary team, including GP Partners, GPs, Advanced Clinical Practitioners, Nurse Practitioners, Paramedic Practitioners, Clinical Pharmacists, Physiotherapists, Practice Nurses, and Healthcare Assistants.
- To conduct face-to-face and telephone consultations, assessing patients' medical conditions, developing personalized care plans, and evaluating patient outcomes.
- To visit patients in their homes to assess their medical condition and frailty review.
- Fully qualified Advanced Clinical Practitioner with experience of working with frail and older patients.
- Full and unrestricted NMC (UK) or HCPC registration.
- RGN (degree or equivalent), Prescribing Qualification, and Qualification in Physical Assessment skills and Frailty.
- Evidence of continuing professional development and recent NHS legislation knowledge.
- Excellent communication and interpersonal skills.
The salary for this role is approximately £45,000 - £60,000 per annum, depending on experience. In addition, you will receive excellent benefits, including access to Employee Assistance Program and the NHS pension.
