About the Role

This pivotal role is responsible for providing clinical pharmacy leadership for the practice, taking responsibility for Structured Medication Reviews, areas of chronic disease management, medicines optimisation, quality improvement, the quality and outcomes framework, and other enhanced services. As a key member of the multi-disciplinary team, you will work alongside experienced pharmacists to provide clinical medication reviews, domiciliary clinical medication review, telephone medicines support, medicine information to practice staff and patients, and unplanned hospital admissions.

Responsibilities
  • Patient-facing clinical medication reviews and domiciliary clinical medication review
  • Telephone medicines support and medicine information to practice staff and patients
  • Unplanned hospital admissions and documents management
  • Repeat prescribing, signposting, information management, education, and training

We require a pharmacist with a minimum of 5 years post-registration experience, including at least 2 years in general practice, certificate in non-medical prescribing, and experience using prescribing qualification in general practice. Successful candidates will be part of a team that meets regularly to train together and learn from each other.
