Our Primary Care Nurse role at Central & North West London NHS Foundation Trust offers an exciting opportunity to work with patients, families, and carers in secure environments, promoting health and wellbeing through compassionate care. As a member of our Health and Justice team at HMP Bronzefield, you will be part of a collaborative approach to healthcare delivery, fostering partnerships between patients, families, and carers, as well as other healthcare professionals.
Key Responsibilities- Triage patient needs based on specific standard operating procedures and national policy
- Implement evidence-based practice to ensure effective and appropriate clinical care
- Develop and maintain strong relationships with patients, families, and carers
- Contribute to the development of integrated healthcare services, ensuring seamless care pathways
To succeed in this role, you must have:
- Registered Nurse Adult qualification
- Numeracy skills for safe medication administration
- Previous experience working in Primary Care services (as a Band 5 Nurse or Student Nurse)
