Nurse Lead Care Coordinator

Job Summary
We are seeking a skilled and experienced Nurse Lead Care Coordinator to join our team at Conexus Healthcare CIC. As a key member of our Primary Care Network (PCN), you will play a vital role in delivering high-quality patient care and improving health outcomes.
Main Responsibilities
- Provide expert nursing care and coordination to patients who are housebound or hard to reach, ensuring they receive the support they need to manage their long-term conditions.
- Lead the roving multi-disciplinary care coordination team, working closely with non-clinical care coordinators, nursing associates, and other PCN staff to deliver seamless patient care.
- Assess patient needs and coordinate the most appropriate care, including referrals to specialist services and community resources.
- Develop and implement personalized care plans, working collaboratively with patients, families, and carers to ensure their needs are met.
- Contribute to the development and implementation of PCN strategies and initiatives, promoting population health and reducing health inequalities.
- Collaborate with local healthcare providers, community organizations, and voluntary sector groups to ensure effective care coordination and patient outcomes.

Requirements
- Qualified Registered General Nurse or equivalent with current NMC registration.
- Substantial post-registration experience in a primary care or community setting.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Ability to work autonomously, prioritizing tasks and managing a caseload.
- Knowledge of clinical governance, audit, and relevant professional and clinical legislation.
- IT skills in Microsoft applications, with experience of using electronic patient records and care planning systems.
