Care Support Specialist

We are seeking a dedicated and compassionate Care Support Specialist to join our team at Cheshire West and Chester Council. As a Care Support Specialist, you will play a vital role in providing high-quality, person-centred care to adults with learning disabilities, autism, dementia, and the elderly.
Our team is committed to maximising the social, emotional, and physical independence of our service users, promoting independence, choice, and control. We achieve this by following individual care plans and risk assessments, ensuring the safety and well-being of our service users.
Key responsibilities of the Care Support Specialist role include:
- Caring for and supporting service users by following care plans and risk assessments to meet their physical, emotional, and mental health needs.
- Keeping full, accurate, and up-to-date records, including risk assessments, care, and support plans for service users and daily records of events while on duty.
- Supporting service users to manage their own emotions and behaviours, encouraging them to do things for themselves, such as dressing, washing, eating, socialising, and managing their finances.
- Administering or supporting with medications.
- Assisting service users with advice and support to help them manage their finances and maintain their homes.
- Acting as a Key Worker, using a person-centred approach, completing monthly reviews of risk assessments, care, and support plans, and contributing to six-monthly reviews.
- In line with care plans, assisting with risk assessments for events/access to the community for service users individually or in groups, leisure facilities, libraries. Supporting service users with activities, such as shopping, days out, swimming.
- Building links within the community to enable service users to access facilities and services, social, leisure, employment, and education.
- Safeguarding service users at all times, taking necessary actions immediately and reporting any concerns or issues to management.
- Working positively with managers, colleagues, and other social workers and health professionals to fully support service users and encourage feedback to improve the care and support provided.
