This is an exciting opportunity to make a positive impact in the lives of adults with complex needs. As a Lead Practitioner, you will provide individualized support to help them achieve their goals and live fulfilling lives.

Our role involves:

  • Providing support to adults with a wide range of needs, either one-to-one or as part of a team.
  • Supporting individuals in the community and engaging with their hobbies and interests.
  • Attending community groups and accompanying service users to activities they enjoy.
  • Assisting with personal care and ensuring their comfort and wellbeing.
  • Celebrating achievements and milestones to create a positive environment.
