Stroke Liaison Nurse Opportunity

We are seeking a highly motivated and experienced nurse to join our stroke team in the Western Isles. This role will involve working closely with the community to provide high-quality stroke care.

Key Responsibilities:

  • Provide expert stroke care to patients in the community
  • Work closely with the multidisciplinary team to develop and implement stroke care plans
  • Conduct regular assessments and reviews to ensure optimal patient outcomes
  • Collaborate with other healthcare professionals to promote stroke awareness and education
  • Develop and maintain strong relationships with community partners to ensure seamless care transitions

Requirements:

  • NMC Registered Adult Nurse
  • Substantial post-registration experience in stroke care
  • Education to degree level or relevant experience at band 5 level
  • Excellent communication, organisation, and time management skills
  • Good understanding and knowledge of policy, procedures, and national targets
