Job Description

This role involves supporting, delivering, and coordinating person-centred dementia care, implementing therapeutic interventions to improve clinical outcomes, and working within a multidisciplinary team of nursing, social care, psychology, and wellbeing practitioners.

  • Conduct holistic nursing assessments and provide general nursing and bio-psychosocial interventions.
  • Share knowledge and deliver best practices, using critical reflective practice to inform decision-making.
  • Operate as an autonomous practitioner, contributing to the development of evidence-based care pathways.
Required Skills and Qualifications

The ideal candidate will have an active NMC Pin, minimum of 2 years' practice experience in a dementia care community, and evidence of delivering person-centred care for people with complex dementia needs.

  • Strong communication skills, flexibility, and creativity, with a commitment to quality care and professional development.
  • Experience in collaborative and multidisciplinary team working, with a proven ability to work autonomously.
