Care and Activities Coordinator

We are seeking a highly motivated individual to join our team as a Care and Activities Coordinator. As a key member of our Magic Moments Team, you will play a vital role in ensuring the wellbeing and engagement of our residents.
Key Responsibilities:
- Assist in the assessment of resident needs through meetings with care staff, family members, and residents themselves.
- Support the development and delivery of stimulating activities programs tailored to meet individual resident needs.
- Build strong relationships with residents, families, and care staff to gather life story information and inform care plans.
- Collaborate with local community groups to arrange trips and excursions that promote social interaction and independence.
- Contribute to the implementation and promotion of our Magic Moments initiatives.
- Evidence and track activity engagement using digital and written methods.
