Job Details:
  • Salary: £25,384 - £27,000 per annum (based on 37.5 hours/week), including a competitive hourly rate of £25.38 - £27 per hour as an umbrella company worker.
  • Location: Reading area with some remote working possible, subject to discussion with management.
  • Contract: Full-time, 12 months long-term vacancy.

About the Role:

We are seeking a skilled and compassionate Band 6 Community Psychiatric Nurse (CPN) or Social Worker to join our Adult Community Mental Health Team (CMHT) in Reading. As a key member of our team, you will be responsible for managing a caseload of up to 30 individuals with diverse mental health needs, ensuring tailored care plans are developed and implemented.

Responsibilities:
  • Caseload Management: Develop and implement personalized care plans for service users, considering their unique needs and circumstances.
  • Care Coordination: Work collaboratively with multidisciplinary teams, families, and service users to deliver holistic care coordination and promote person-centered support.
  • Diverse Cases: Provide interventions that cater to varying severities of mental health conditions, focusing on recovery and resilience.
  • System Expertise: Utilize electronic record systems effectively to document and track care provided.
  • Autonomy and Teamwork: Balance independent work with collaborative efforts within a supportive team environment.
Requirements:
  • Qualifications: Registered Mental Health Nurse (CPN) or qualified Social Worker with valid professional registration.
  • Experience: Proven experience managing caseloads within an Adult CMHT or equivalent setting.
  • Skills: Strong communication, organizational, and problem-solving abilities.
  • IT Proficiency: Familiarity with electronic record systems.
  • Driving: Must have access to a car and hold a valid driver's license.
  • Flexibility: Some part-remote working may be possible, subject to discussion with management.
