We are seeking an experienced Social Worker to join our team at Randstad Care, partnering with the Pathways Leaving Care Team in Middlesbrough to support young people leaving care.

About the Role:
  • Manage a caseload of young people leaving care, ensuring their needs are assessed and addressed through a comprehensive needs assessment process.
  • Develop and implement individualized Pathway Plans that promote independence, well-being, and positive outcomes for care leavers.
  • Provide support and guidance to care leavers in accessing education, employment, housing, and other essential services, fostering a smooth transition to adulthood.
  • Collaborate with multi-disciplinary teams, external agencies, and other professionals to coordinate services and resources, ensuring a cohesive approach to care leaver support.
  • Ensure compliance with relevant legislation, policies, and procedures related to leaving care and child protection, maintaining the highest standards of practice.
About the Team:

The Pathways Leaving Care Team at Randstad Care is dedicated to providing high-quality support services to young people leaving care, empowering them to achieve their full potential.

Requirements:
  • Qualified Social Worker registered with Social Work England, with a strong understanding of the leaving care process.
  • Significant experience in a social work role, particularly working with care leavers or young people in care, with a proven track record of delivering positive outcomes.
  • Excellent communication and interpersonal skills, with the ability to build effective relationships with young people, families, and professionals.
  • Knowledge of relevant legislation, policies, and procedures related to leaving care and child protection, with a commitment to ongoing professional development.
