About the Role

We are seeking an experienced and skilled Advanced Nurse Practitioner to join our team at Wyre Forest Health Partnership. As a key member of our primary care team, you will be responsible for delivering high-quality patient care and contributing to the development of our practice.

Key Responsibilities
  • Assess, diagnose, plan, implement, and evaluate treatment/interventions and care for patients presenting with undifferentiated diagnoses.
  • Treat patients wishing to see a healthcare professional, making necessary referrals to other members of the primary healthcare team.
  • Ensure clinical practice is safe and effective, remaining within the boundaries of competence and acknowledging limitations.
  • Record accurate and legible notes of all consultations and treatments.
  • Prescribe safe, effective, and appropriate medication, adhering to current legislative frameworks and guidelines.
  • Liaise with doctors regarding patients with complex problems requiring general practitioner expertise.
Requirements
  • Self-directed practitioner with excellent communication skills.
  • Highly motivated and flexible individual with enthusiasm for teamwork.
  • Ability to work across boundaries and articulate the rationale for decisions.
  • Clinical examination skills and experience in general practice (desirable).
