Healthcare Coordination Professional

We are seeking an experienced Healthcare Coordination Professional to join our team at Care New England Health System. In this role, you will work closely with interdisciplinary teams to coordinate patient care and ensure seamless transitions between hospital and home settings. Your expertise in developing individualized treatment plans, monitoring progress, and identifying areas for improvement will be invaluable in enhancing patient outcomes.

About the Role:
  • Develop and implement comprehensive treatment plans tailored to each patient's needs
  • Collaborate with healthcare professionals to coordinate care and monitor progress
  • Conduct regular assessments and evaluations to identify areas for improvement
  • Communicate effectively with patients, families, and healthcare teams to ensure clear understanding of care plans and progress


Estimated Salary: $80,000 - $100,000 per year.

Requirements:
  • Bachelor's Degree in a relevant field
  • A minimum of two years progressive clinical experience
  • Valid RI Board licensure
  • Valid driver's license and automobile liability insurance
