We are looking for a dedicated SEN Support Worker to join our team at Austin Fraser. As an experienced caseworker, you will be responsible for coordinating and managing EHC plans for children and young people with complex needs.

**About the Role:**

This role requires excellent communication and interpersonal skills, as well as the ability to work effectively with families, education settings, and a range of health and social care professionals. The successful candidate will also have a strong understanding of special educational needs and the SEND Code of Practice 0-25.

**Key Responsibilities:**

  • Conduct and coordinate EHC needs assessments and manage EHC plans in line with statutory requirements.
  • Liaise and build positive relationships with families, educational institutions, and professionals across education, social care, and health sectors.
  • Support families in understanding and accessing services to best meet their child's needs.
