About the Role

We are seeking a highly skilled and experienced Case Worker to lead our team in providing effective and efficient liaison between the CHC Clinical staff and care providers. The successful candidate will have a strong background in case management and be able to work independently, with a high level of autonomy.

Key Responsibilities
  • Liaise with placements, monitor care provision, and changes to presentation which may affect an individual's eligibility.
  • Provide accurate and effective communications within and outside of the CHC service, dealing professionally and discreetly with enquiries from patients, relatives, carers, legal representatives, health & adult social care professionals, and other organisations.
  • Undertake reporting and analysis of information to support delivery and development of the role.
  • Meet deadlines, collate data, and maintain standards relating to the Key Performance Indicators and Quality.
Requirements
  • Advanced keyboard skills, use of a range of software.
  • Knowledge of administrative procedures, project management or information analysis.
  • Knowledge of commissioning care provision and contractual agreements and the roles & responsibilities of agencies providing these.
  • Working knowledge of Microsoft Office.
  • An ability to pull together draft reports, data and other correspondence.
  • Ability to pull together comprehensive draft reports, data and letters. Negotiating, networking, and persuasive skills.
