Job Title: Homelessness Inclusion Health Nurse

We are seeking a highly skilled and compassionate Homelessness Inclusion Health Nurse to join our team in Watford. As a key member of our outreach team, you will be responsible for providing high-quality healthcare services to individuals experiencing homelessness.

Main Responsibilities:
  • Develop therapeutic alliances with individuals experiencing homelessness using trauma-informed care principles.
  • Undertake specialist homeless healthcare at an advanced level, including holistic physical and psychosocial health assessments.
  • Act as a health case manager for individuals with high levels of unmet health need.
  • Proactively identify immediate clinical risks to life and limb, and provide necessary clinical/therapeutic primary care treatments on the street.
  • Ensure good communication of any treatments offered to other healthcare agencies.
  • Identify barriers to housing and work to get individuals into housing solutions.
  • Develop and maintain close working relationships with homeless and inclusion health services.
Requirements:
  • Experience of working as a clinical practitioner in an inclusion health setting.
  • Broad knowledge and experience covering primary care, acute, mental health, and addictions.
  • Ability to independently represent the service at local and national levels.
  • Experience of directly delivering nurse-led health street outreach.
  • Understanding of how nurse-led health street outreach fits into the wider infrastructure.
  • Good understanding of legal literacy in respect to housing, immigration, and healthcare access.
What We Offer:
  • A competitive salary range of £43,814.48 (pro rata for part-time hours).
  • A fixed-term contract until February 2026.
