Job Title: Street Healthcare Outreach Nurse

We are seeking a compassionate and skilled Street Healthcare Outreach Nurse to join our team at Change Grow Live. As a key member of our outreach team, you will provide high-quality clinical services to individuals experiencing homelessness, addressing their physical and mental health needs.

About the Role:
  • Provide healthcare and clinical interventions to individuals in need, working closely with our outreach team to identify and address health concerns.
  • Develop and implement individualized care plans, taking into account the unique needs and circumstances of each service user.
  • Collaborate with other healthcare professionals and community organizations to ensure comprehensive support for our service users.
  • Contribute to the development and implementation of innovative clinical interventions, driving positive outcomes for individuals experiencing homelessness.

Requirements:
  • Registered Nurse with a strong background in community health and outreach services.
  • Experience working with vulnerable populations, including individuals experiencing homelessness.
  • Excellent communication and interpersonal skills, with the ability to build trust and rapport with service users.
  • Ability to work independently and as part of a team, with a flexible and adaptable approach to meeting the needs of our service users.
What We Offer:

We offer a competitive salary range of £43,814.50 - £48,183.48 (pro rata for part-time hours), plus Outer Fringe allowance of £683.99 (pro rata for part-time). This is a fixed-term contract until February 2026, providing a stable and secure work environment.
