About the Role

At Livewell Southwest CIC, we are looking for a skilled mental health professional to lead our learning disability services. The ideal candidate will have experience in working with individuals with complex needs and a passion for delivering high-quality care.

The estimated salary for this position is £45,000 - £65,000 per annum.

Main Duties
  1. To assess and plan care for individuals with learning disabilities, taking into account their physical, mental health, and behaviour that challenges needs.
  2. To provide case management and care coordination for individuals who are fully health funded.
  3. To liaise with commissioners and other stakeholders to ensure effective service delivery.
  4. To speak up for individuals with learning disabilities and ensure they receive equitable healthcare.
  5. To liaise with other agencies, providers, and families to promote safeguarding and support.

Clinical Leadership
  • Participate in health needs reviews and assessments to identify needs and plan care.
  • Identify, plan, and manage risk of an individual's care needs, relating to physical health, behavioural, and or mental health.
  • Responsible for assessing, planning, and making recommendations for future care in partnership with patients, carers, and multi-professional agencies.
  • Liaise with Primary and Secondary care agencies to ensure better health outcomes for individuals.
  • Maintain effective communication networks with members of the multi-disciplinary team and other stakeholders.
