About the Role

We are seeking an experienced and skilled Clinical Lead Continence to join our Community Continence Team. As a key member of our team, you will be responsible for providing professional nursing leadership and guidance to ensure the delivery of high-quality care to our patients.

Key Responsibilities

  • Manage and coordinate the care of patients with complex continence needs, ensuring that their individual needs are met and their care is delivered in a timely and effective manner.
  • Develop and implement care plans that are tailored to the needs of each patient, working closely with other healthcare professionals to ensure that care is coordinated and effective.
  • Provide leadership and guidance to a team of healthcare professionals, ensuring that they have the skills and knowledge needed to deliver high-quality care.
  • Monitor and evaluate the quality of care delivered, making recommendations for improvement as needed.
  • Work closely with other teams and services to ensure that patients receive seamless care and support.
